• From 15 years direct referrals are received from Child Safety.
• Post care (18 - 20 years) young people can self-refer or with support from another organisation.
Expectations • It is expected that a Transition Officer should have 12-15 young people on their caseload at a time.
• Young people that are receiving support need to have a clear exit pathway, which focuses on establishing and strengthening community connections, referring to long-term support services if assessed, and building skills to minimise housing vulnerability.
• Interconnections with other specific transition from care programs (Specialist Services Team, DCSYW, and non-government organisations including Next Step Plus) are strengthened and provide an integrated service response for young people with a disability who are at risk of homelessness as they transition to adulthood.
Eligibility, referral pathway and expectations will be monitored through the performance measures in each reporting period.
