Tender: GPACI Aged Care Coordination Grant for General Practices — Busselton, Bunbury, Geraldton, Albany

WA Primary Health Alliance is seeking applications from general practices in Busselton, Bunbury, Geraldton and Albany for a GPACI grant to improve coordination of primary care for residents of residential aged care homes (RACHs). The grant will fund practices to build aged care coordination capacity by establishing a new role or expanding an existing role to support practical day-to-day activities, including using practice systems, templates, recalls and reminders to meet GPACI and care planning requirements; coordinating appointments, GP and nurse practitioner visits, case conferences and telehealth sessions; coordinating information exchange after hospital presentations and discharges to support continuity of care; and identifying and implementing practical improvements to aged care workflows. Practices must also identify and implement at least two sustainability initiatives to support continuation of coordination activities beyond the funding period. Applications will be assessed against predefined criteria such as alignment with grant objectives, anticipated impact for aged care residents, implementation approach and sustainability. There is no minimum grant amount and practices may apply for up to $100,000. Issued by WA Primary Health Alliance.
