About
WA Country Health Service awarded a contract to GP down south (GP DOWN SOUTH LTD) for the face-to-face delivery of diabetes education in Manjimup and Bridgetown.
The Grant Agreement required provision of education for clients with Type 1 Diabetes (T1DM), Type 2 Diabetes (T2DM), pre-diabetes and gestational diabetes (GDM), and followed the vision of the Australian National Diabetes Strategy 2021 to “strengthen, integrate and coordinate all sectors to improve health outcomes and reduce the social and economic impact of diabetes in Australia.”
The award engaged a local primary care provider to deliver in-person diabetes education in regional communities, reflecting a focus on integrated, community-level services aligned with the national diabetes strategy.
