About
Mental Health Commission awarded a contract to Parkerville Children and Youth Care Inc (PARKERVILLE CHILDREN & YOUTH CARE INC) for the development and implementation of a WA Head to Health (H2H) Kids Hub in the City of Swan.
The service targeted infants and children aged 0-12 with mild to moderate emerging complexity and provided comprehensive, multidisciplinary care to support children and their families.
The hub aimed to improve early intervention outcomes for children's mental health and wellbeing and to complement and enhance existing child health and family services in the catchment.
Delivery was required to align with the WA Service Model developed from the Head to Health Kids National Service Model, and the Commission appointed a single service provider to deliver a locally tailored H2H Kids Hub integrated with existing local services.
