NSW Health is seeking registered Support at Home (SAH) providers (categories 1-5) to deliver interim, time-limited home-based care equivalent to Level 7 and Level 8 SAH packages for older people who are clinically ready for discharge from NSW public hospitals but are awaiting Commonwealth SAH package allocation or reassessment. Services must support safe, timely discharge and maintenance of client health, wellbeing and independence, reduce avoidable hospital bed days, improve patient flow across selected NSW Local Health Districts, and be delivered consistently across metropolitan, regional and rural locations while supporting the NSW Government Aged Care Response Package and achieving value for money, strong clinical governance and positive client outcomes.
