Department of Health (Royal Street) awarded a contract to WA AIDS Council Inc (Western Australian AIDS Council (Inc)) for community based case management support services for people living with HIV/AIDS.
The service uses a bio-psychosocial model of mobile community case management to provide intensive support to 24-26 clients with chaotic and transient lives, including working with medical teams to encourage compliance with HIV medication, mental health, sexual health, and other health matters.
Support also covers housing, substance use, finances, legal, and relationship issues, with emphasis on Aboriginal and culturally and linguistically diverse (CALD) clients.
