About
Lifetime Care and Support Scheme is seeking suppliers for the new intake to the Approved Case Manager and Care Needs Assessor panel (Scheme SCM11991). Approved Case Managers provide specialised services across the rehabilitation continuum—from early post-discharge rehabilitation programs to lifelong support—to help people with very severe disabilities and complex circumstances achieve community participation and quality of life outcomes. Approved Care Needs Assessors are qualified professionals who conduct specific assessments of participants of the Lifetime Care and Support Scheme and the Workers Care Program to identify care and support requirements specific to the participant’s injury type and severity; these assessments are utilised by Lifetime Care and Worker’s Care to make and communicate decisions regarding funding, support and attendant care service requirements for participants.
