About
Department of Health (Royal Street) awarded a contract to Access Care Network Australia for continuity of support (COS) for Home and Community Care Program (HACC) clients who were ineligible for the National Disability Insurance Scheme (NDIS).
The contract continued to support existing COS clients, assisted clients with their NDIS applications, guided clients eligible for NDIS to transition to alternate providers, and ensured COS services were not ceased until new services were in place with alternate providers.
The award focused on maintaining uninterrupted care during clients' NDIS transitions to reduce the risk of service gaps for HACC clients.
