Specialised Support Services for Young People with Intellectual or Learning Disabilities Transitioning from Statutory Care
Awarded 01 Jul 2024
About
DCSSDS awarded a contract to Community Living Assn Inc for specialised services and support for young people aged 15-20 with an intellectual or learning disability who are homeless or at risk of homelessness, transitioning to adulthood, and have been or are currently subject to statutory intervention.
The service will work with each young person to plan a clear exit pathway by age 21, focusing on establishing and strengthening community connections, building skills to minimise housing vulnerability, and advocacy or referral to long-term support services if required.
The service will prioritise referrals for young people with a disability who may not receive support through the NDIS and need assistance to develop skills needed to maintain safe and stable housing as they transition to adulthood.
Prioritised referrals may come from Child Safety for young people from 15 years or post care young people (18-20 years) can self-refer or request support via another organisation.
The service will maintain interconnections with other care-transition programs (Specialist Service Team – DCYJMA, and non-government organisations) to strengthen an integrated service response for the target group.
The contract value is AUD 821,920, procured via limited method.
