Award of Specialised Transition and Post Care Support Services for Young People with Disabilities
Awarded 31 Dec 2028
About
DCSSDS awarded a contract to Life Without Barriers for specialised services and support to young people with an intellectual or learning disability who are homeless or at risk of homelessness and transitioning to adulthood, including those subject to statutory intervention.
The program, Community Living Association – Transition and Post Care Support, provides support to eligible young people aged 15-20 (transitioning out by 21) with a disability or complex needs, prioritizing those at risk of homelessness who may not receive NDIS support.
Referrals come from Child Safety from age 15, and self-referrals or referrals from other organisations for post-care young people aged 18-20.
Transition Officers manage caseloads of 12-15 young people, focusing on clear exit pathways that establish community connections, refer to long-term support services, and build skills to minimise housing vulnerability.
The service strengthens interconnections with other transition from care programs, including Specialist Services Team, DCSYW, and NGOs such as Next Step Plus, to provide an integrated response.
The contract value is AUD 3,526,892, procured via limited method.
