DCSSDS awarded a contract to Open Minds Australia Limited for specialized service and support to young people with a disability who are homeless or at risk of homelessness and transitioning to adulthood, including those subject to statutory intervention.

The program, Open Minds - Transition and Post Care Support - Disability, targets young people aged 15-20 with a disability or complex needs, providing case management with a Transition Officer caseload of 12-15 young people, and focuses on developing skills to maintain safe and stable housing, establishing clear exit pathways, and strengthening community connections.

Referrals come from Child Safety from age 15, and young people aged 18-20 can self-refer or be referred by other organizations.
